After specifying information about yourself and your patient, MedicAlert Foundation® will relay your patient’s medical information over the phone. All information from MedicAlert is patient provided and may include:

  • Past Medical History
  • Allergies
  • Medications
  • Family History
  • Immunizations
  • Emergency Contacts
  • Health Insurance

Furthermore, MedicAlert can fax other important documents to the hospital or medical facility where you are providing care for your patient. Examples of documents that may be faxed to you are:

  • Emergency Medical Information Record (EMIR®)
  • Advance Directives
  • EKG Report
  • Clinical Trial Information
  • Difficult Intubation Report
  • Implanted Device Card
  • Treatment Protocols
  • Pre-Hospital DNR

Since 2007, the MedicAlert + Safe Return® program has been the link between law enforcement and Alzheimer’s Association chapters as well as patient caregivers to bring wandering missing patients home. If they find a missing person with dementia, or are searching for a missing person with dementia, law enforcement officers are provided with information over the phone and via fax. Including:

  • Patient identification information such as name, age, height, weight, tattoos, eye color, hair color, etc
  • Patient’s last known location
  • Caregiver contact information
  • Primary language
