During a medical emergency use the login you created during registration to gain access to your patient’s MedicAlert® Emergency Medical Information Record (EMIR®).

You will need to provide a few pieces of information before you access your patient's record: MedicAlert® member ID, Facility of Care, and Reason for Accessing Record.
MedicAlert Member ID
Enter your patient’s MedicAlert® member ID number. This can be found engraved on the back of your patient’s medical ID jewelry or on their MedicAlert® issued wallet card.
Enter the hospital at which your patient is receiving care. If you are EMS on your way to a hospital please enter the destination hospital name. Enter your patient's MedicAlert® member ID number. This can be found engraved on the back of your patient’s medical ID jewelry or on their MedicAlert® issued wallet card.
Reason for Accessing Record
Enter the reason you are accessing this patient’s record from the list of options such as "patient needs medical care".
Verify that you indeed have the correct patient’s record such as photo, gender, and age. If not, cancel and re-enter your patient's MedicAlert® member ID number.
The PrEMISE® app will indicate a Missing Person alert if the person has been reported missing by their caregiver or by the Alzheimer's Association. You will need to call MedicAlert Foundation®.
Press on the Missing Person icon to take you to the next screen where you will be able to call MedicAlert with one touch button.
Emergency Medical Information
View the patient's vital emergency medical information. We store a wide array of information as provided by the member such as Advance Directives, Allergies, Medications, Past Medical, Emergency Contact information, and Health Insurance, plus more.
View your patient's address and phone number as well as your patient's emergency contact phone number(s).
View health insurance information as provided by the patient.
View pertinent documents the patient has stored with MedicAlert Foundation such as: DNR orders, EKG reports, Advance Directives, or treatment protocols.
Fax this emergency record along with any pertinent stored documents to your healthcare facility. You will need to enter the fax number.
